Pharmacy application
High school students applying for the Pharmacy partnership program must fill out the Ursuline College and University of Toledo Application for Admission. (Requires Acrobat Reader. Download Acrobat Reader free.)
Students must also submit:
- $65 application fee
- Official transcripts from high school and any college attended
- ACT or SAT scores
- A letter of recommendation from guidance counselor or teacher
- A personal statement: (choose one of the following or choose a topic of your own)
- Describe a significant life experience.
- A woman who has inspired me.
- The reason for choosing my major.
Students may print out the application from the above link, or call Ursuline College at 1.888.URSULINE or 440.449.4203 to request an application. Completed applications, along with the $65 application fee, should be mailed to the following address:
